چکیده

Although extensive research has been done to improve the treatment of vitiligo, an universal effective treatment for vitiligo is not yet available [1]. PUVA therapy has demonstrated to have immunomodulatory effects, mainly in the T lymphocyte compartment but has several adverse effects, such as skin cancer risk [2,3]. The optimization of PUVA therapy is a relevant clinical objective in the management of vitiligo patients. Polypodium leucotomos (PL) extract has demonstrated to have relevant photoprotective effects in humans [4,5] and animal models [6]. These biological effects may be involved in the action mechanism of PL on vitiligo patients, and could explain the enhancement of the pigmentary response of conventional PUVA by PL when this was used as adjuvant treatment in vitiligo patients [4]. In order to optimize PUVA treatment for management of vitiligo patients, we therefore investigated potential immunomodulatory effects of PUVA associated with PL. For this purpose, a pilot randomized, double-blind, placebo-controlled, clinical trial designed to study effects of PUVA + PL on T lymphocytes from 19 generalized vitiligo patients was conducted (Table 1). At baseline (before first PUVA session) and after 12 weeks of treatment (before of the last PUVA session), peripheral blood both from patients and from each corresponding control was withdrawn. Repigmentation response was evaluated

A comparison between systemic PUVA therapy alone and combined with topical Calcipotriol in the treatment of generalized Vitiligo

Background: Vitiligo is a relatively common acquired depigmentary disorder affecting approximately 1% of general population. PUVA therapy is the most effective modality and commonly used as the primary treatment of generalized Vitiligo. Objective: To evaluate the synergistic effect of topical Calcipotriol and oral methoxalene plus PUVA in treatment of generalized Vitiligo. Patients and Me...

Polypodium leucotomos as an Adjunct Treatment of Pigmentary Disorders.

INTRODUCTION Extracts of the tropical fern Polypodium leucotomos appear to possess beneficial properties for the skin attributed to the presence of numerous compounds within the extract that have antioxidant and photoprotective properties. Orally administered Polypodium leucotomos may provide protection against the detrimental photoaging effects of sunlight and can also help reduce the frequenc...

Orally administered Polypodium leucotomos extract decreases psoralen-UVA-induced phototoxicity, pigmentation, and damage of human skin.

BACKGROUND The use of psoralen-UVA (PUVA) in patients of skin phototype I to II is limited by side effects of acute phototoxicity and possible long-term carcinogenesis. OBJECTIVE We sought to assess oral Polypodium leucotomos (PL) extract in decreasing PUVA-induced phototoxicity of human skin on a clinical and histologic level. METHODS A total of 10 healthy patients with skin phototypes II ...

A comparison between systemic psoralen plus ultraviolet A therapy and topical clobetasol in the treatment of vitiligo

Background: Topical corticosteroids and oral psoralen plus ultraviolet A radiation (PUVA therapy) are two common treatment methods for vitiligo. The aim of this study was to compare the efficacy, complications and patterns of repigmentation of these two methods. Methods: This prospective randomized clinical trial was conducted from July 2007 to October 2008. Patients with disseminated vitiligo ...

Current and emerging therapy for the management of vitiligo

Vitiligo is an acquired cutaneous disorder of pigmentation, with an incidence of 0.5% to 2% worldwide. There are three major hypotheses for the pathogenesis of vitiligo that are not exclusive of each other: biochemical/cytotoxic, neural and autoimmune. Recent data provide strong evidence supporting an autoimmune pathogenesis of vitiligo. As vitiligo can have a major effect on quality of life, t...
